The folio number is assigned by the Property Appraiser's office and is different from the legal description — you need both. Go to miamidade.gov/pa and search by property address to find the folio number.

Miami-Dade County-Specific Form Requirements
- Property Folio Number: Required. Format: XX-XXXX-XXX-XXXX. Find it at miamidade.gov/pa.
- Building Permit Number: Required for most project types.
- Contractor's State License Number: Full DBPR license number required.
- 3-inch top margin on first page, 1-inch on all other sides.
- Minimum 8-point font for legibility.

How to File in Miami-Dade County
- Prepare the NOC including folio number, permit number, contractor license number, and proper margins.
- Have the owner sign before a Florida notary (or use Notary Connect for remote signing).
